• You are submitting a claim for death pension benefits.

By clicking on the above link you will open the Department of Veterans Affairs (DVA) home page. As you scan to the far right you will find an entry titled "Quick Links", under which you locate "VA Forms." By selecting VA Forms, you will find a blank entry for "Form Number" enter 21-534 and enter search. This will bring up the forms for you to select. The best form to use is the first: 21 534a(PDF).
